The service allows customers to request the issue of an official certificate upon the death of a person. The death certificate will state the date, place, nationality and religion of the deceased.

Submit the application via the e-system and enter the notification number (QAID number) to retrieve the information
Upload the Deceased Burial Permit issued by the police into the system
Pay fees if required.
Receive the certificate through delivery or by visiting the public health center in the Emirate
The death certificate stamped by the Department of Health in Abu Dhabi or Dubai Health Authority or Emirates Health Services is equivalent to the stamp of the MOHAP. The certificate might need attestation from Ministry of Exterior and the embassies.
The certificate fee is AED 65 for Arabic and AED 65 for English certificate (if requested).
